Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with combining kava and pazopanib (Votrient). This isn't about kava changing how much pazopanib is in your body. Instead, it's about the liver. Pazopanib is already known to be tough on the liver, and it carries a warning about liver injury that doctors watch closely with regular blood tests. Kava has its own history of rare but serious liver damage, mostly with heavy or long-term use.

Stacking two things that can stress the liver may raise your risk together. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. To be fair, this caution is based mostly on anecdotal reports, not strong studies. Still, please check with your oncologist or pharmacist before using kava.

P-GLYCOPROTEIN SUBSTRATES

Pazopanib also falls under P-GLYCOPROTEIN SUBSTRATES.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist for Votrient (pazopanib). Pazopanib is carried around your body partly by a transporter called P-glycoprotein, which acts like a pump that moves the drug back out of cells. There's a theory that kava might block this pump, which could let more pazopanib build up than expected.

If that happened, you might see stronger side effects from pazopanib, things like high blood pressure, diarrhea, fatigue, or changes in liver tests. I want to be honest, though: this concern comes mainly from lab studies, and a human study using kava didn't change levels of a similar pump-dependent drug. So it's possible but far from proven. Since kava itself can stress the liver (just like pazopanib can), please run this by your oncologist or pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Pazopanib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with pazopanib (Votrient). Pazopanib is cleared from your body mainly by a liver enzyme called CYP3A4. The worry on paper is that kava might slow that enzyme down, which could let pazopanib build up and bump up side effects like high blood pressure, fatigue, diarrhea, or liver strain.

The good news: this concern comes from lab dish studies, and a real human study found kava had no meaningful effect on CYP3A4. So an actual interaction is rated unlikely and minor. The bigger issue is that both kava and pazopanib can stress the liver on their own. Because pazopanib treats a serious cancer, please don't add kava without first checking with your oncologist or pharmacist.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
