P-GLYCOPROTEIN SUBSTRATES

Pomalidomide falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with kava and your pomalidomide (Pomalyst). Pomalidomide is moved around the body partly by a transporter called P-glycoprotein, which acts like a tiny pump pushing drugs out of cells. In lab dishes, kava seemed to slow that pump down, and if that happened in real life, more pomalidomide could linger in your system. With this cancer medicine, that could mean stronger side effects like low blood counts, fatigue, infections, or a higher clot risk.
But the evidence here is thin. The lab signal didn't show up in a human study using digoxin, another P-gp drug. So this is more theoretical than proven. I'd still loop in your oncology team before adding kava, partly because of its rare liver risks. Please don't change anything on your own.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version: your body uses an enzyme called CYP1A2 to help break down pomalidomide. There was some early lab work and a single case report hinting that kava might slow that enzyme down. If that were true, it could theoretically let pomalidomide build up a bit, which might mean more of its usual side effects like low blood counts, fatigue, or a higher infection risk.
But the better-quality human study tells a calmer story. When healthy volunteers took kava for 28 days, it did not meaningfully change CYP1A2 activity. So this interaction is unlikely and considered minor. The bigger concern with kava is its rare link to liver damage, which matters a lot during cancer treatment. Please loop in your oncology pharmacist or doctor before adding kava.

What I'd genuinely watch instead is your liver. Both kava and pomalidomide can stress the liver, so combining them isn't something I'd do casually. Please loop in your oncology team or pharmacist before adding kava.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.
About Pomalidomide
What Is Pomalidomide Used For?
Brand name: Pomalyst
Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.
Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
