Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing to know about pairing kava with nirogacestat (Ogsiveo): this isn't about one changing the blood level of the other. It's about the liver. Kava has been tied to rare but real cases of liver injury, and nirogacestat can also raise liver enzymes and stress the liver in some people. Stacking two things that lean on the liver may add up.

I'll be honest that this concern is theoretical and based mostly on scattered case reports, not solid studies. Still, since your care team likely checks your liver labs while you're on nirogacestat, it's worth a quick chat before adding kava. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please loop in your pharmacist or doctor first.

Nirogacestat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this pairing. Nirogacestat (Ogsiveo), the medication used to treat desmoid tumors, is processed by a liver enzyme called CYP3A4. On paper, if kava slowed that enzyme down, nirogacestat could build up and you might see more of its typical side effects like diarrhea, nausea, mouth sores, or fatigue.

But in practice, this worry hasn't panned out. Test-tube studies hinted kava might block CYP3A4, yet a real study in healthy people taking kava for a month showed no meaningful effect on that enzyme. So an interaction here is unlikely. The bigger concern with kava is actually its rare link to liver injury, which matters since nirogacestat can already affect the liver. Please loop in your oncology team or pharmacist before combining them.

Brand name: Ogsiveo

Ogsiveo (nirogacestat) is used to treat adults with desmoid tumors that are growing or progressing and require systemic (whole-body) treatment. Desmoid tumors are rare, locally aggressive growths that arise from connective tissue.

This indication is specific to Ogsiveo tablets taken by mouth. It is not approved for use in children, as safety and effectiveness have not been established in pediatric patients.
