Gabapentin enacarbil falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to know about pairing kava with Horizant (gabapentin enacarbil): both of them calm down your nervous system. Horizant already tends to make people feel sleepy, dizzy, or a little foggy, and kava works in a similar relaxing way on the brain. Stack them together and those effects can pile up, so you may feel more drowsy, unsteady on your feet, or slowed down than you'd expect from either one alone.

This is a well-supported concern, and expert groups specifically warn against mixing kava with sedating medicines. There's also kava's separate risk of liver injury to keep in mind. I wouldn't combine these on your own. Please check with your pharmacist or doctor first, and be extra careful with driving until you know how you feel.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Gabapentin

What Is Gabapentin Used For?

Brand name: Horizant

Gabapentin (brands include Neurontin, Relgaabi, Gabarone, Gralise, and Horizant) is primarily used to manage postherpetic neuralgia (PHN) — the nerve pain that lingers after a shingles outbreak — in adults. Neurontin, Relgaabi, Gabarone, and gabapentin capsules, tablets, and oral solution are also used as add-on therapy for partial onset seizures (a type of epilepsy) in adults and children aged 3 and older.

Horizant (gabapentin enacarbil extended-release tablet) has two specific approvals: treatment of moderate-to-severe primary restless legs syndrome (RLS) in adults, and management of postherpetic neuralgia in adults. Horizant is not recommended for people who need to sleep during the day and stay awake at night. Gralise is approved only for postherpetic neuralgia and cannot be substituted for other gabapentin products due to its different release profile.
