Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with combining kava and pexidartinib (Turalio): both can be tough on your liver. Pexidartinib already carries a serious warning for liver injury, which is why people taking it get regular blood tests to watch their liver enzymes. Kava has been tied to its own rare cases of liver damage, mostly with heavy or long-term use. Stacking the two could, in theory, add to that stress on your liver.

To be honest, this is based on anecdotal reports rather than strong studies, so it's not a guaranteed problem. Still, with a medication this hard on the liver, I'd play it safe. Please don't start, stop, or adjust anything on your own. Just check with your oncology team or pharmacist before adding kava.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the honest picture: your body clears pexidartinib (Turalio) partly through a liver enzyme called CYP3A4. Some early lab studies hinted that kava might slow that enzyme down, which in theory could let pexidartinib build up a bit. But the better, real-world evidence tells a different story. In a study of healthy people taking 1,000 mg of kava twice daily for nearly a month, kava had no measurable effect on CYP3A4. So this interaction is rated minor and unlikely.

My bigger concern with this pairing is the liver itself. Both kava and pexidartinib can cause serious liver injury on their own, so combining them isn't something I'd do casually. Please check with your oncology team or pharmacist before adding kava, and watch for yellowing skin, dark urine, or unusual fatigue.
