Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and Polivy (polatuzumab vedotin-piiq). This isn't about kava changing how Polivy works in your body. Instead, it's about your liver. Polivy can stress the liver and raise your liver enzymes, and kava has been tied to rare but serious liver injury, mostly with heavy or long-term use. Stacking two things that can both tax the liver may add up.

I'll be honest: this is a theoretical concern based on case reports, not strong studies. Still, since you're already going through intensive lymphoma treatment with bendamustine and rituximab, I'd skip kava unless your oncology team okays it. Please loop in your doctor or pharmacist before adding it.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this one: the worry about kava and Polivy comes from the fact that the active cancer-killing piece of this medicine (called MMAE) is partly broken down by a liver enzyme named CYP3A4. In theory, if kava blocked that enzyme, the drug could build up and cause stronger side effects like low blood counts, nerve tingling or numbness, or infection risk. But that's mostly a lab-dish concern. In a real human study, kava taken daily for a month did not actually change CYP3A4 activity, so a meaningful interaction is unlikely.

The bigger flag with kava is its own rare risk of liver damage, which matters a lot during cancer treatment. Please loop in your oncology team or pharmacist before adding kava.

What Is Polatuzumab vedotin-piiq Injection Used For?

Brand name: Polivy

  • Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
