Why this applies to Flibanserin

Flibanserin falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to know if you take Addyi (flibanserin) and are thinking about kava. Flibanserin already has a calming, sedating side to it, which is one reason it's taken at bedtime. Kava works in a similar direction, gently quieting brain activity. Put the two together and those effects can stack up, leaving you feeling unusually drowsy, dizzy, lightheaded, or slow to react.

This isn't a far-fetched worry. Good evidence supports kava adding to the sedation of other calming substances, and major guidelines advise against mixing kava with these kinds of drugs. With flibanserin, low blood pressure and fainting are already a concern, so extra sedation could make that worse. Please check with your pharmacist or doctor before combining them, and never stop your medication on your own.

Flibanserin also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the situation with kava and Addyi (flibanserin). Lab studies suggest kava may slow down a liver enzyme called CYP2C19, which is one of the pathways your body uses to clear flibanserin. If that pathway is blocked, the drug can build up in your system. Flibanserin isn't a prodrug, so more of it generally means stronger effects, mainly dizziness, drowsiness, fainting, and low blood pressure.

I'll be honest, this is a theoretical concern so far, seen only in test-tube research and not confirmed in people. Still, there's an extra reason to be careful here: both kava and flibanserin can stress the liver, and flibanserin already carries warnings about sedation. Please check with your pharmacist or doctor before combining them.

Flibanserin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with Addyi (flibanserin). Flibanserin is cleared by a liver enzyme called CYP3A4. The theory is that kava might slow that enzyme down, which could let more flibanserin build up and make side effects like dizziness, sleepiness, low blood pressure, or even fainting more likely. The good news is that the better-quality human research suggests kava actually has little or no real effect on CYP3A4, so this concern is mostly lab-based and rated as minor and unlikely.

That said, I'd still be thoughtful here. Both kava and flibanserin can have calming, sedating effects on their own, so stacking them may leave you more drowsy or lightheaded. Kava has also been linked to rare liver damage, which is its own reason for caution. Please check with your pharmacist or doctor before pairing these.

What Is Flibanserin Used For?

Brand name: Addyi

Addyi is approved to treat acquired, generalized hypoactive sexual desire disorder (HSDD) — a condition where low sexual desire causes significant distress or relationship difficulty — in women under 65. "Acquired" means the low desire developed after a period of normal desire; "generalized" means it happens regardless of the situation or partner.

Addyi is not intended for use in men, and it is not a sexual performance enhancer. It is also not meant for HSDD that is caused by a medical condition, a psychiatric condition, relationship problems, or another medication.
