Flupenthixol falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Flupenthixol (Fluanxol) is an antipsychotic that already has calming, sedating qualities. The concern here is simple math: kava also quiets the nervous system, so stacking the two can add up. Together they may leave you feeling more drowsy, foggy, or slowed down than either one alone, with effects like sleepiness, dizziness, sluggish reflexes, and trouble concentrating becoming more noticeable.

This isn't about kava changing how your body processes the drug. It's an additive effect, and the evidence behind it is strong enough that expert guidelines say to avoid pairing kava with calming medications. There's also kava's separate risk of liver harm to keep in mind. Please don't change anything on your own. Talk with your pharmacist or doctor before combining these.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist for flupenthixol (Fluanxol) and kava. Your liver uses an enzyme called CYP2D6 to help clear flupenthixol from your body. In test-tube studies, kava looked like it might block that enzyme, which in theory could let flupenthixol build up and bring on more of its usual effects, like drowsiness, dizziness, restlessness, or muscle stiffness. The good news: when researchers actually tested this in people, kava did not meaningfully change how the body handles CYP2D6 medications.

So this concern is mostly theoretical and rated unlikely. A bigger real-world worry is that both kava and flupenthixol can be sedating, and kava has been tied to rare liver injury. Please check with your pharmacist or doctor before combining them.
