Why this applies to Proguanil

Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's something worth knowing about proguanil (Paludrine), which you likely take to prevent malaria. Proguanil is actually a prodrug. Your body uses an enzyme called CYP2C19 to convert it into its active form, cycloguanil, which is the part that does the real work against the malaria parasite.

Lab studies suggest kava may block that CYP2C19 enzyme. In theory, that could mean less of your proguanil gets switched on, which might make it work less well for preventing malaria. I want to be honest though: this has only been seen in test-tube research, not in actual people, so we don't truly know if it matters in real life. Since malaria prevention is too important to gamble with, please run this combination by your pharmacist or doctor before traveling.
