Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.

Here's a wrinkle worth knowing if you take Proguanil (Paludrine) for malaria. Proguanil is actually a prodrug, meaning your body has to convert it into its active form, cycloguanil, and the enzyme CYP2C19 does most of that conversion work. Lab studies suggest resveratrol may slow down (inhibit) that same enzyme.

So in theory, resveratrol could blunt how well Proguanil gets activated, which might mean weaker malaria protection rather than more side effects. The important caveat: this is based only on test-tube research and has never actually been seen in people, so it's far from proven. Still, when you're relying on a medicine to prevent malaria, you don't want to gamble. Mention any resveratrol supplement to your pharmacist or doctor before your trip.
