Proguanil falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C19 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to inhibit CYP2C19 activity in vitro.

Here's an interesting wrinkle with Proguanil. It's actually a prodrug, meaning your liver has to convert it into its active form, called cycloguanil, using the enzyme CYP2C19. That active form is what does most of the work against the malaria parasite.

Adrafinil turns into modafinil in your body, and modafinil may slow down that same CYP2C19 enzyme. In theory, that could mean less of your Proguanil gets switched into its active form, which might make it a bit less effective at preventing or treating malaria. I want to stress this is theoretical, based only on lab data, not real patient reports. Still, with something as important as malaria protection, it's worth running past your pharmacist or travel doctor before combining them.
