Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the simple version: lurbinectedin (Zepzelca) can be tough on your liver on its own, and the same goes for kava. Kava has been tied to more than 100 reports of liver injury, mostly with heavy or long-term use. Stacking two things that both stress the liver could, in theory, raise your chances of liver trouble while you're on chemo.

I'll be honest, this is mostly a caution based on anecdotal reports rather than strong studies, so we don't want to overstate it. Still, your care team already watches your liver labs closely with lurbinectedin, and signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness matter. Please loop in your oncologist or pharmacist before using kava during treatment.

Lurbinectedin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses an enzyme called CYP3A4 to help clear lurbinectedin (Zepzelca) out of your system. Some early lab studies hinted that kava might block that enzyme, which in theory could let this chemo drug build up and cause stronger side effects like low blood counts, nausea, fatigue, or a higher infection risk. But here's the reassuring part: when researchers actually tested kava in people, it did not meaningfully change CYP3A4 activity. So this is rated a minor, unlikely interaction.

That said, kava carries its own warning for rare liver injury, and lurbinectedin is serious chemotherapy. I'd never add kava during cancer treatment without first running it by your oncology team or pharmacist.

What Is Lurbinectedin Injection Used For?

Brand name: Zepzelca

Zepzelca (lurbinectedin) injection is approved for adults with small cell lung cancer (SCLC) in two settings. First, Zepzelca is used in combination with atezolizumab (or atezolizumab and hyaluronidase-tqjs) as maintenance treatment for extensive-stage small cell lung cancer (ES-SCLC) in patients whose disease did not progress after initial treatment with those same immunotherapy drugs plus carboplatin and etoposide.

Second, Zepzelca is approved as a single agent for adults with metastatic small cell lung cancer whose cancer has worsened during or after platinum-based chemotherapy. This second use was granted accelerated approval based on tumor response rates, and continued approval depends on results from confirmatory clinical trials.
