Why this applies to Mavacamten

Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the situation with these two. Mavacamten (Camzyos) is cleared from your body partly by a liver enzyme called CYP2C19. Lab studies suggest kava can block that enzyme. If that blocking actually happened in people, your body might clear mavacamten more slowly, letting levels creep up. With this particular heart medication, higher levels are a real concern because too much can weaken the heart's pumping (a drop in what doctors call ejection fraction), leaving you short of breath, tired, dizzy, or with swelling.

I want to be straight with you though: this is only seen in test-tube research so far, never confirmed in actual patients. On top of that, kava itself can be hard on the liver. Because Camzyos is closely monitored with regular heart scans, please don't add kava without first checking with your cardiology team or pharmacist.

Mavacamten also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this pairing. Your heart medicine Camzyos (mavacamten) is cleared from the body partly by a liver enzyme called CYP3A4. Some early test-tube studies hinted that kava might slow that enzyme down, which in theory could let mavacamten build up and increase effects like a weaker heart squeeze, dizziness, or shortness of breath. But that worry hasn't held up in people. In a real human study, kava at a solid daily dose didn't actually change CYP3A4 activity at all.

So this interaction is rated minor and unlikely. Still, mavacamten is a drug that needs careful dose monitoring, and kava carries its own liver risks, so please check with your pharmacist or doctor before adding it.
