Nelfinavir Mesylate falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the gist if you take kava alongside your HIV medicine nelfinavir (Viracept). In test-tube studies, kava appears to slow down a liver enzyme called CYP2C19, which helps clear nelfinavir from your body. If that same effect happened in people, nelfinavir levels could creep up a bit, which in theory might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset. Nelfinavir is an active drug (not a prodrug), so blocking that enzyme would tend to raise its levels, not weaken it.

Honestly, this is only seen in lab research so far, never confirmed in real people, so don't panic. But because kava can also stress the liver, I'd run it by your pharmacist or doctor before combining them.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the practical picture with kava and your HIV medicine, Viracept (nelfinavir). Nelfinavir relies partly on a transporter called P-glycoprotein, which acts like a doorman moving the drug in and out of cells. Lab studies hint that kava might slow that doorman down, which could in theory let a bit more nelfinavir build up in your body. That might mean more of its usual side effects, like diarrhea, nausea, gas, or stomach upset.

The good news: this is weak, mostly test-tube evidence, and a human study with another P-gp drug showed no real effect. So a meaningful problem is uncertain. Still, kava carries its own risk of liver harm, which matters with HIV meds. Please check with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with nelfinavir (Viracept). Nelfinavir is processed by a liver enzyme called CYP3A4, and there was a theory that kava might slow that enzyme down. If that were true, nelfinavir could build up and cause more side effects like diarrhea, nausea, or gas. But the better human research doesn't back this up. In a real study, kava taken daily for a month had no meaningful effect on CYP3A4, so this interaction is unlikely to cause problems.

My bigger concern is your liver. Kava has been tied to rare but serious liver injury, and HIV care already asks a lot of your liver. Keeping nelfinavir working steadily matters for controlling HIV, so please check with your pharmacist or doctor before adding kava.
