Here's the gist for your HIV medication. When your liver turns adrafinil into modafinil, that modafinil can gently slow down a liver enzyme called CYP2C19, which is one of the pathways that helps clear nelfinavir from your body. If that enzyme works slower, nelfinavir levels could drift a bit higher, which in theory might mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset.

I want to be honest about how soft this evidence is. This interaction is only theoretical and comes from lab studies, not from people, so it may not amount to much in real life. Still, keeping your HIV treatment steady and predictable matters a lot. Please don't change anything on your own. Have a quick chat with your doctor or pharmacist before pairing adrafinil with nelfinavir, especially since adrafinil is unregulated and can stress the liver.

Here's the situation if you're thinking about pairing adrafinil with nelfinavir (Viracept). Once adrafinil reaches your liver, it gets turned into modafinil, the active wakefulness drug. Modafinil is cleared in part by a liver enzyme called CYP3A4. Nelfinavir is a strong blocker of that same enzyme, so in theory it could slow the breakdown of modafinil and let it build up higher than expected.

If that happens, you might feel more of modafinil's effects: trouble sleeping, jitteriness, headache, a racing or pounding heart, or anxiety. I'll be honest with you, this is a theoretical concern based on how these drugs work, not something proven in studies. Still, since nelfinavir is treating HIV and adrafinil is an unregulated supplement that's hard on the liver, please check with your doctor or pharmacist before mixing them.

Here's the gist for your HIV medicine. Once your liver turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4. Nelfinavir (Viracept) is cleared in part by that same enzyme, so in theory adrafinil could nudge your body to break nelfinavir down a little faster, leaving lower levels in your blood.

Why that matters: with HIV treatment, you want steady, reliable drug levels. If nelfinavir drops too low, the virus could be controlled less well over time. I want to be honest, though, this is a theoretical concern based on how these drugs work, not strong real-world proof, and modafinil's effect on this enzyme is considered weak.
