These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Adrafinil interacts with whole categories of medication. Avatrombopag Maleate falls into 2 of those categories, so it can interact with Adrafinil in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
1 of 2Why this applies to Avatrombopag Maleate
Avatrombopag Maleate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your platelet medicine, Doptelet (avatrombopag). Your liver uses an enzyme called CYP2C9 to help clear this drug. Adrafinil, once your body turns it into modafinil, may slow that enzyme down a bit. In theory, that could let avatrombopag build up higher than intended, which might nudge your platelet count up more than expected.
Why does that matter? With too many platelets, the worry is a higher risk of blood clots. I want to be honest though: this is a theoretical concern based on lab findings, not real patient reports, so I wouldn't lose sleep over it. Since avatrombopag is usually dosed carefully around a procedure, please loop in your doctor or pharmacist before combining these so they can keep an eye on things.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Avatrombopag Maleate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Doptelet (avatrombopag): your liver uses an enzyme called CYP3A4 to help clear this medicine. Adrafinil turns into modafinil in your body, and modafinil can gently rev up that same enzyme. In plain terms, your body may break down avatrombopag a little faster, which could lower its levels and make it less effective at boosting your platelet count.
Why that matters: avatrombopag is prescribed to raise platelets and lower bleeding risk before a procedure or for low-platelet conditions. If its effect weakens, you might not get the platelet bump you need. I'll be honest though, this concern is theoretical, based on how the drugs work rather than real patient reports, and modafinil is only a weak inducer. Still, please loop in your doctor or pharmacist before combining them.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Avatrombopag Maleate page
“Avatrombopag maleate (Doptelet) exists to raise a low platelet count, so anything that drags its blood level down invites the bleeding risk it was prescribed to prevent. I would handle this list of 54 supplements with real care, and the two rated major come first.”
