What Avatrombopag is used for

Avatrombopag treats low platelet counts (thrombocytopenia) in these situations:
- Doptelet tablets: adults with chronic liver disease who are scheduled to have a procedure.

How Avatrombopag works
▾
Avatrombopag is a small-molecule medicine that attaches to the thrombopoietin (TPO) receptor on cells in the bone marrow. This signals megakaryocytes, the cells that make platelets, to grow and mature. The result is more platelets in the blood.
It doesn't compete with your body's own TPO. Instead, it adds to TPO's effect.
How Avatrombopag is taken (dosage)
▾
Avatrombopag is taken by mouth once daily with food. Doptelet tablets are swallowed. Doptelet Sprinkle capsules are opened and the granules mixed into a soft food or liquid and taken right away.
- Chronic liver disease (tablets): a 5-day course that starts 10 to 13 days before the procedure. The dose depends on your platelet count, and all 5 days should be finished.
- ITP: the dose is adjusted based on regular platelet counts, with the goal of keeping platelets at a safe level, not a normal one.
- The two forms are not interchangeable mg for mg. Follow your prescriber and the label.

For platelet count less than 40×10 9 /L, the dose is 60 mg (3 tablets) orally once daily; for platelet count 40 to less than 50×10 9 /L the dose is 40 mg (2 tablets) orally once daily.

2 DOSAGE AND ADMINISTRATION DOPTELET tablets and DOPTELET SPRINKLE are not substitutable on a mg-to-mg basis. DOPTELET SPRINKLE capsules should be opened, and the contents (oral granules) mixed with a soft food or liquid. Administer immediately after mixing. Do not swallow the capsules whole. Administer DOPTELET tablets and DOPTELET SPRINKLE with food. ( 2.1 , 2.3 , 2.5 ) Chronic Liver Disease : Dose DOPTELET tablets based upon platelet count prior to procedure, orally for 5 days beginning 10 days to 13 days before procedure. For platelet count less than 40×10 9 /L, the dose is 60 mg (3 tablets) orally once daily; for platelet count 40 to less than 50×10 9 /L the dose is 40 mg (2 tablets) orally once daily. ( 2.2 ) Adult Patients with Chronic Immune Thrombocytopenia and Pediatric Patients 6 Years and Older with Persistent or Chronic Immune Thrombocytopenia : Initiate DOPTELET tablets at 20 mg (1 tablet) orally once daily. Adjust the dose or frequency of dosing to maintain platelet count greater than or equal to 50×10 9 /L. Do not exceed 40 mg (2 tablets) per day. Pediatric Patients 1 Year to Less than 6 Years with Persistent or Chronic Immune Thrombocytopenia : Initiate DOPTELET SPRINKLE oral granules at 10 mg (content of 1 capsule) orally once daily. Adjust the dose or frequency of dosing to maintain platelet count greater than or equal to 50×10 9 /L. Do not exceed 20 mg (content of 2 capsules) per day. 2.1 Important Use and Administration Instructions Select the recommended product (DOPTELET tablets or DOPTELET SPRINKLE) based on the indication and patient’s age. Administer DOPTELET tablets and DOPTELET SPRINKLE with food. DOPTELET tablets and DOPTELET SPRINKLE are not substitutable on a mg-to-mg basis. The mixture prepared from the granules in DOPTELET SPRINKLE capsules is more bioavailable than DOPTELET tablets [see Clinical Pharmacology ( 12.3 )] . There is no experience from clinical trials in switching between dosing with the granules and the tablet. If the formulation is switched, monitor platelet counts weekly until stable platelet counts are obtained and adjust dosing as needed before resuming monthly monitoring. 2.2 Recommended Dosage of DOPTELET Tablets for Patients with Chronic Liver Disease Begin DOPTELET tablets dosing 10 days to 13 days prior to the scheduled procedure.

- Call your doctor if you have unusual bleeding or bruising that worsens.
- Call your doctor if you think you took too much. Too high a platelet count can lead to clots.

Avatrombopag warnings and precautions

Blood clots are the biggest risk. Avatrombopag belongs to a drug class linked to thrombotic and thromboembolic complications (clots). Your doctor will check platelet counts and watch for clot symptoms.
Extra care is needed if you have a clotting disorder or other risk factors for clots. It should not be used to push platelet counts into the normal range.
Avatrombopag interactions: what it interacts with

Certain medicines that affect two liver enzymes, CYP2C9 and CYP3A4, change avatrombopag levels in people with ITP.
- Moderate or strong dual inhibitors of CYP2C9 and CYP3A4: raise avatrombopag levels and may increase side effects, so a lower starting dose is used.
- Moderate or strong dual inducers of CYP2C9 and CYP3A4: lower avatrombopag levels and may reduce how well it works, so a higher starting dose is used.

- There are no listed contraindications (conditions that rule out use).

- Pregnancy: it may cause fetal harm based on animal studies. Discuss this with your doctor.

- Children: tablets are established for ages 6 and older, and Doptelet Sprinkle for ages 1 to under 6, for ITP.
Avatrombopag overdose: if you take too much

Too much avatrombopag can push platelet counts too high and cause blood clots. Platelet counts and symptoms are monitored closely, and clots are treated by standard care. There is no known antidote. Contact Poison Help (1-800-222-1222) or a medical toxicologist.

In adults, platelet counts rose in line with the dose and exposure. The increase started within 3 to 5 days, peaked after 10 to 13 days, and slowly returned to near baseline after treatment stopped.
