Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version on combining kava with ondansetron (Zofran). On paper, kava was thought to maybe slow down a liver enzyme called CYP1A2, which helps clear ondansetron from your body. If that were true, ondansetron could build up a bit and cause more of its usual effects, like headache, constipation, or feeling tired.

But here's the reassuring part: the better-quality research found that kava actually had no meaningful effect on CYP1A2. So this interaction is rated minor and unlikely, and it's really more of a theoretical concern than something you'll notice. My bigger caution with kava is its rare link to liver damage, so it's always worth a quick chat with your pharmacist or doctor before using it regularly.

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with ondansetron (Zofran): the worry on paper is that kava might slow down an enzyme called CYP2D6, which is one of the pathways your body uses to clear ondansetron. If that happened, you could in theory end up with a bit more ondansetron hanging around, which might mean side effects like headache, constipation, or feeling tired.

The good news is that this stays mostly theoretical. Lab dish studies showed kava blocking that enzyme, but actual human studies did not find kava changing how CYP2D6 medicines are handled. Ondansetron also uses several enzymes, not just one, so it's fairly forgiving. The bigger thing to watch with kava is its rare liver risk. Run it by your pharmacist or doctor first.

Here's the short version: ondansetron (Zofran) is partly cleared by a liver enzyme called CYP3A4, and there was a theory that kava might slow that enzyme down. If that were true, ondansetron could build up a bit and you might notice more of its usual mild effects, like headache, constipation, or feeling tired. But the better-quality human research tells a different story. In one study, healthy volunteers took a real kava dose for 28 days and it did not change CYP3A4 activity.

So in practice, this interaction looks unlikely to cause problems. The bigger thing to keep in mind with kava is its rare risk of liver damage, so always loop in your pharmacist or doctor before combining the two.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
