Ibuprofen, Hydrocodone falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Ibucodone is a combo pill, and the part that matters here is the hydrocodone, an opioid pain reliever that already slows down your brain and breathing. Kava has its own calming, sedating effect on the nervous system. Stack the two together and those effects can add up. That can mean more drowsiness, dizziness, foggy thinking, slowed reflexes, and in a worst case, dangerously slow or shallow breathing.

This isn't just a lab theory. Expert guidelines specifically warn against mixing kava with sedating medicines, so I'd treat this seriously. There's also a separate concern: both kava and ibuprofen can be hard on the body (kava on the liver, ibuprofen on the stomach and kidneys). Please don't start kava with this medication before checking with your pharmacist or doctor first.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the deal with combining kava and your ibuprofen/hydrocodone combo (Ibucodone). The concern on file is that kava might block an enzyme called CYP2D6, which your body uses to turn the hydrocodone part into its active pain-relieving form. In a test tube, kava did slow this enzyme down, but here's the reassuring part: actual human studies showed kava did not meaningfully change how this enzyme works in people. So this interaction is rated minor and unlikely.

The more practical worry is overlap in effects. Kava is calming and hydrocodone is a sedating opioid, so together they could leave you feeling extra drowsy, dizzy, or foggy. Kava can also stress the liver, and so can frequent ibuprofen use. Please check with your pharmacist or doctor before pairing these.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on the metabolism side: the hydrocodone portion of this combo is processed by a liver enzyme called CYP3A4, and people worried that kava might block it. But the better-quality human studies show kava really doesn't change CYP3A4 much at all. So I wouldn't expect kava to make your hydrocodone build up or wear off faster through this pathway. The interaction on file here is rated minor and unlikely.

That said, there's a more practical concern. Both kava and hydrocodone can make you drowsy, foggy, or relaxed, so stacking them could leave you more sedated than you'd like. Kava has also been tied to rare liver injury, and ibuprofen is hard on the stomach. Please run this combination by your pharmacist or doctor before mixing them.
