Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the simple version: your cough syrup contains codeine, which is an opioid that already makes a lot of people feel drowsy, foggy, and slowed down. Kava also has a calming, sedating effect on the brain. Put the two together and those effects can stack on top of each other, which is what we call an additive interaction.

The real concern is being extra sleepy, dizzy, slow to react, or even having slowed, shallow breathing, since opioids can quiet your breathing drive. Major expert guidelines flatly recommend not mixing kava with sedating medicines like this one. Please don't start or stop anything on your own. Just check with your pharmacist or doctor first, especially since kava can also stress the liver.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the thing with this combo. Codeine is what we call a prodrug, meaning your body has to switch it on. A liver enzyme called CYP2D6 converts codeine into morphine, which is the part that actually relieves your cough and pain. Some lab (test tube) studies hinted that kava might block CYP2D6. If that happened in real life, you'd theoretically get less pain and cough relief from the codeine, not more. The guaifenesin part isn't really affected.

The reassuring news is that actual human studies showed kava did not change how the body handles CYP2D6 drugs. So this is mostly a paper concern. The bigger reason to check in with your pharmacist is that both kava and codeine can make you drowsy, and kava carries a rare risk of liver harm.
