Aspirin, Codeine Phosphate, Methocarbamol falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

This combination pill contains two ingredients that already calm your nervous system: codeine (an opioid pain reliever) and methocarbamol (a muscle relaxer). Both can make you sleepy and slow your reflexes on their own. Kava works in a similar quieting way on the brain, so stacking it on top can pile up that sedation.

Together, you might feel unusually drowsy, foggy, dizzy, or unsteady on your feet, and in stronger cases your breathing and reaction time can slow down. There's another reason to be careful here: both kava and aspirin can be tough on the liver and stomach, and kava has been linked to rare but serious liver injury. Expert guidelines actually advise against mixing kava with sedating medicines, so please check with your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the interesting wrinkle with this combination. The codeine portion of Robaxisal C is a prodrug, meaning your body uses an enzyme called CYP2D6 to turn it into morphine, which is what actually relieves pain. In the test tube, kava looked like it might block that enzyme, which could in theory leave you with weaker pain relief. But here's the reassuring part: actual human studies showed kava did not meaningfully change how the body handles CYP2D6 drugs. So this interaction is rated minor and unlikely.

My bigger real-world concern is overlap. Both kava and codeine can make you drowsy, foggy, or slow your breathing, so stacking them can add up. Kava also carries a rare liver risk. Please run this combo by your pharmacist or doctor before mixing them.
