Selumetinib Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your child's medication, selumetinib (Koselugo), is cleared from the body partly by a liver enzyme called CYP3A4. In a test tube, kava looked like it might block that enzyme, which in theory could let selumetinib build up and cause more of its usual side effects, things like rash, diarrhea, nausea, tiredness, or fluid buildup. But that's only part of the story.

When researchers actually tested kava in real people for 28 days, it had no meaningful effect on CYP3A4. So this concern is mostly theoretical and unlikely to cause trouble. Still, kava carries its own real risk of liver injury, which matters with any serious medication. I'd run kava past your child's oncology team or pharmacist before adding it.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
