Selumetinib Sulfate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know: this interaction is really about bleeding risk. Danshen seems to thin the blood and slow clotting, and that effect can stack on top of medicines that already affect platelets or clotting. Selumetinib itself isn't a blood thinner in the usual sense, so the connection here is more general than drug-specific.
Still, selumetinib (Koselugo) can cause its own problems like nosebleeds, mouth sores, and stomach upset, and bleeding is something to watch for in kids on this medicine. Adding danshen could, in theory, make easy bruising or bleeding more likely. Honestly, the evidence is thin and mostly anecdotal or from animal studies, so I don't want to alarm you. Please check with your child's oncology team or pharmacist before using danshen.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Selumetinib Sulfate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's what matters for your child's Koselugo (selumetinib). Your body relies on a liver and gut enzyme called CYP3A4 to break this medicine down. Danshen can nudge that enzyme in either direction. When taken regularly for a week or two, it seems to speed the enzyme up, which could lower selumetinib levels and make it less effective at slowing those nerve tumors. But a single dose appears to do the opposite, raising drug levels, which could mean more side effects like rash, diarrhea, mouth sores, or fatigue.
Selumetinib is not a prodrug, so this is simply about how much active drug stays in the body. The evidence is early and a bit mixed, so please don't start or stop anything. Just check with your oncology team or pharmacist first.

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).
Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
