Aspirin, Codeine Phosphate falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

The piece to focus on here is the codeine in this combo. Codeine is an opioid that calms down your central nervous system, and kava has its own calming, sedative-like effects. When you stack the two together, those effects can add up. That means you could feel more drowsy, foggy, or unsteady than usual, and in larger amounts there's a real concern about slowed breathing, which is the most serious risk with any opioid.

This isn't a vague theory. Expert guidelines specifically warn against mixing kava with sedating medications, so I'd treat it seriously. Please don't start, stop, or change either one on your own. Have a quick chat with your pharmacist or doctor first so they can look at your full picture, including the fact that kava can also stress the liver.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the thing worth knowing about your aspirin with codeine combo: the codeine part is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. So anything that blocks that enzyme could mean less pain relief, not more.

In a test tube, kava looked like it might block CYP2D6. But when researchers studied this in real people, kava didn't actually change how CYP2D6 medications were handled. So in practice, this is rated minor and unlikely to cause problems. My bigger concern is overlap in drowsiness, since both kava and codeine can make you sleepy. Both can also stress the liver. Please check with your pharmacist or doctor before combining them.
