Ibuprofen, Oxycodone falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Combunox combines ibuprofen with oxycodone, and it's the oxycodone part that matters most here. Oxycodone is an opioid that already slows down the brain, causing drowsiness and slower breathing. Kava has its own calming, sedative-like effects. Put them together and those effects can stack up, which can leave you feeling more groggy, dizzy, or sluggish than expected.

The bigger worry is that both can slow your breathing and dull your reflexes. That combination raises the risk of dangerously slow breathing, especially at higher doses. This is a well-supported concern, and expert guidelines specifically advise against mixing kava with these kinds of medicines. Please don't change anything on your own, but do check with your pharmacist or doctor before using kava with this medication.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Let's talk about combining kava with this combo pill, which pairs ibuprofen (an anti-inflammatory) with oxycodone (an opioid pain reliever). On paper, there's a worry because lab studies hint that kava might slow down an enzyme called CYP2D6. Oxycodone is partly handled by that enzyme, so in theory blocking it could shift how much active drug is around.

Here's the reassuring part: in actual human studies, kava did not meaningfully change how the body processes CYP2D6 drugs. So this particular interaction is rated minor and unlikely. The bigger real-world concern is overlap in sedation, since both kava and oxycodone can make you drowsy, plus kava's rare liver risk alongside ibuprofen. Please check with your pharmacist or doctor before mixing them.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

This combo product pairs ibuprofen with oxycodone, a strong opioid painkiller. The theory here is that kava might block CYP3A4, the liver enzyme that helps break down oxycodone. If that happened, oxycodone could build up and cause more drowsiness, dizziness, slowed breathing, or constipation. But the better human research is reassuring: a study giving people kava daily for a month found no real effect on CYP3A4, so this enzyme worry is mostly theoretical.

What I'd watch more closely is the overlap in sedation. Both kava and oxycodone calm the brain, so together they may make you extra sleepy or foggy. Kava also carries a rare risk of liver harm. Please check with your pharmacist or doctor before combining them.
