Aspirin, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Preliminary clinical research suggests that taking capsules containing a combination of astragalus, goldthread, and Baikal skullcap daily for 4 weeks inhibits platelet aggregation; the effect seems to be similar to that of aspirin 50 mg daily. It is unclear if this effect is due to Baikal skullcap, other ingredients, or the combination.

Here's the thing to keep in mind with this combo: your medication contains aspirin, which already thins the blood and makes it harder for platelets to clump and form clots. Some early lab work hints that Baikal skullcap may have a mild blood-thinning, anti-platelet effect of its own. Stack the two together and, at least in theory, you could nudge your bleeding risk a little higher.

What might that look like? Easier bruising, nosebleeds, bleeding gums, or a cut that's slow to stop. I'll be honest, though, this concern comes mostly from test-tube and animal studies, not solid human data, so it's a soft signal rather than a proven problem. The codeine part isn't really involved here. Before adding the supplement, just run it by your pharmacist or doctor.

Preliminary clinical research has not identified clinically relevant sedation after use of Baikal skullcap; however, a thorough evaluation of safety outcomes has not been conducted.

The codeine part of this combination is a painkiller that can make you feel drowsy or relaxed. Baikal skullcap has been studied in test tubes and animals, where it seems to gently nudge the same calming brain receptors (called GABA-A) that many sedating medicines act on. So the theory is that taking them together could add up to a bit more sleepiness, slower thinking, or feeling foggy.

Here's my honest take: this concern rests on lab and animal work, and early human studies haven't shown meaningful sedation from the herb. The risk is rated as minor and unlikely. Still, if you notice extra drowsiness, dizziness, or trouble staying alert, ease back and check in with your pharmacist or doctor before combining them.
