Why this applies to Rifampin

Rifampin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might alter the levels and clinical effects of OATP substrates.
Some pharmacokinetic research shows that baicalin, a constituent of Baikal skullcap, can decrease plasma levels of rosuvastatin. The mechanism is thought to involve stimulation of the activity of the organic anion-transporting polypeptide 1B1 (OATP1B1), which transports rosuvastatin into the liver. This decreases plasma levels of the drug, but increases levels at the site of action in the liver. The degree to which rosuvastatin levels are affected depends on the OATP1B1 haplotype of the individual. Baikal skullcap might also affect other OATP1B1 substrates.

Here's the gist: rifampin gets pulled into your liver by a transporter called OATP1B1. Lab and early human studies suggest baicalin, a compound in Baikal skullcap, might give that transporter a little nudge, moving more rifampin into the liver and slightly lowering the amount floating in your bloodstream. Rifampin isn't a prodrug, so this would mean a touch less drug in circulation, not more.

Honestly, this is mostly theoretical and based on weak evidence (and on rosuvastatin, not rifampin specifically), so I wouldn't lose sleep over it. Still, TB and meningitis treatment depends on steady, reliable drug levels, so don't add Baikal skullcap without first checking with your pharmacist or doctor.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
