Aspirin, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the wrinkle with this combo: codeine is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is the part that actually relieves pain. Terminalia arjuna may block that enzyme, so less codeine gets converted. In plain terms, you could get less pain relief than expected, not more.

I want to be honest about how shaky this is. The concern comes only from lab (test tube) studies, not from people, so it's theoretical. The aspirin side isn't affected by this enzyme. Still, if you take Terminalia regularly and notice your pain medicine working less well, mention it to your pharmacist or doctor before changing anything.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the thing to know about pairing Terminalia (often Terminalia arjuna) with your aspirin and codeine combo. The aspirin part is what matters most here. Aspirin already thins the blood by making platelets less sticky, which is exactly the same effect researchers have seen with Terminalia in lab studies. Stack the two and you may, in theory, nudge your bleeding risk a little higher.

In real life that could look like easier bruising, nosebleeds, bleeding gums, or stomach upset that lasts longer than usual. I'll be honest with you though, this concern comes only from test-tube and animal research, not solid human studies, so the risk is more theoretical than proven. The codeine side isn't really involved. Before adding Terminalia, just run it by your pharmacist or doctor, especially if you take aspirin daily.
