Why this applies to Nifedipine
Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses an enzyme called CYP3A4 to break down nifedipine. Lab studies suggest Terminalia arjuna may slow that enzyme down. If that happens in people, nifedipine could clear more slowly and build up a bit, leading to stronger effects than expected. With a blood pressure medicine, that might show up as dizziness, lightheadedness, flushing, headache, swollen ankles, or your blood pressure dropping too low.
The good news is this is only theoretical so far, based on test-tube and animal research, not on real patients. So I don't want you to panic. Still, since both touch the heart and blood pressure, please run this combination by your pharmacist or doctor before starting Terminalia.

Nifedipine treats vasospastic angina (chest pain caused by a spasm in a heart artery) and chronic stable angina (chest pain that comes on with effort). It can be used for chronic stable angina when other treatments, such as beta blockers or nitrates, haven't worked well enough or can't be tolerated.
Nifedipine extended-release tablets, including Procardia XL, are also used to treat high blood pressure. The nifedipine capsule label covers the angina uses only.
