Doxorubicin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the idea behind this pairing. Your body uses a liver enzyme called CYP3A4 to help break down doxorubicin. Lab studies (done in test tubes, not in people) suggest Terminalia arjuna may slow that enzyme down. If it did the same thing in your body, doxorubicin could clear out more slowly and build up a bit, which in theory could mean stronger effects and more side effects like nausea, mouth sores, low blood counts, or added strain on the heart.

I want to be honest about the strength of this: it's theoretical and based only on early lab work, so we don't really know if it matters in real life. Still, with a chemo drug like this one, I'd play it safe. Please check with your oncology team or pharmacist before adding Terminalia.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. About Doxorubicin

What Is Doxorubicin Used For?

Brand names: Doxil, Adriamycin, Rubex, Caelyx, Myocet

Standard (non-liposomal) doxorubicin products, including Adriamycin, are used with other chemotherapy after surgery for women with breast cancer that has spread to nearby lymph nodes. They are also used to treat acute lymphoblastic leukemia, acute myeloblastic leukemia, Hodgkin lymphoma, and non-Hodgkin lymphoma. Other uses include several metastatic (spread) cancers: breast, Wilms’ tumor, neuroblastoma, soft tissue sarcoma, bone sarcoma, ovarian, bladder, thyroid, stomach, and lung (bronchogenic).

Liposomal doxorubicin products, including Doxil, have different uses. They treat ovarian cancer that has progressed after platinum-based chemotherapy, and AIDS-related Kaposi’s sarcoma after other chemotherapy failed or was not tolerated. They also treat multiple myeloma, in combination with bortezomib, in people who have not had bortezomib and have had at least one prior therapy.
