Why this applies to Dexamethasone
Dexamethasone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down dexamethasone and clear it out. Lab studies suggest that Terminalia arjuna may slow that enzyme down. If that happens in real life, dexamethasone could hang around longer and build up a bit, which might make its effects and side effects stronger. With a steroid, that can mean things like trouble sleeping, mood swings, higher blood sugar, fluid retention, or stomach upset.
That said, I want to be honest about the evidence. This is based only on test-tube research, not on people, so it's mostly theoretical. I wouldn't panic, but I also wouldn't pair them on your own. Please run it by your pharmacist or doctor first.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. What Is Dexamethasone Used For?

Dexamethasone is mainly used to calm inflammation and overactive immune responses. Depending on the product, it treats allergic conditions, skin diseases, adrenal and other hormone disorders, and certain bowel diseases.
Some uses belong only to specific products. Hemady tablets are used with other anti-myeloma medicines for adults with multiple myeloma. Dextenza (an eye insert) treats eye inflammation and pain after eye surgery and eye itching from allergic conjunctivitis. Ozurdex (an eye implant) treats macular edema after retinal vein occlusion, non-infectious uveitis of the back of the eye, and diabetic macular edema. Maxidex and dexamethasone sodium phosphate eye drops treat steroid-responsive inflammation of the front of the eye.
