Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the gist with Rexulti (brexpiprazole) and Terminalia. Your body relies on a liver enzyme called CYP2D6 to break down brexpiprazole. Lab studies suggest that Terminalia arjuna may slow this enzyme down. If that happens in real life, brexpiprazole could clear out of your system more slowly and build up a bit, which may make its effects and side effects stronger. With this drug, that could mean more drowsiness, restlessness or an urge to keep moving, dizziness, or weight changes.

I want to be honest though: this is based only on test-tube research, not on people, so it's more of a theoretical concern than a proven problem. Still, since brexpiprazole is a serious medication, please run it by your pharmacist or doctor before combining the two.

Lab studies suggest that Terminalia arjuna may slow that enzyme down. Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
