Terminalia + Diazepam
Diazepam brand names: Valium, Valium injection, Diastat AcuDial, Valtoco.
Diazepam falls into 2 drug categories that Terminalia can interact with; the most serious is rated moderate. Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Terminalia interacts with whole categories of medication. Diazepam falls into 2 of those categories, so it can interact with Terminalia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
1 of 2Why this applies to Diazepam
Diazepam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your body uses a liver enzyme called CYP2C9 (along with a few others) to help clear diazepam out of your system. Lab studies suggest that Terminalia arjuna may slow that enzyme down. If that happens, diazepam could hang around a bit longer than usual and its effects could feel stronger.
For diazepam, that might show up as extra drowsiness, dizziness, grogginess, or feeling unsteady on your feet. The good news is this is only a theoretical concern, based on test-tube research rather than studies in people, so it may not amount to much in real life. Still, I'd mention it to your pharmacist or doctor before combining them, especially if you already notice diazepam makes you sleepy.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Diazepam also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your liver uses an enzyme called CYP3A4 to help break down diazepam (Valium). Lab studies suggest that Terminalia arjuna may slow that enzyme down a bit. If diazepam clears more slowly, it can build up in your system, which could mean stronger or longer-lasting effects like extra drowsiness, dizziness, feeling foggy, or being more wobbly on your feet.
Now, I want to keep this in perspective. This concern comes only from test-tube research, not from studies in actual people, so it's mostly theoretical at this point. Still, since diazepam already causes sedation, it's worth a quick chat with your pharmacist or doctor before combining the two, especially if you're older or take other calming medications.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Terminalia
What is Terminalia?
- Heart health and circulation
- Digestive problems (constipation, diarrhea)
- Cholesterol support
- Antioxidant and anti-inflammatory support
- Immune support
Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. Talk to your pharmacist or doctor before using it, especially if you have a heart condition.
About Diazepam
What Is Diazepam Used For?
Brand names: Valium, Valium injection, Diastat AcuDial, Valtoco
Diazepam treats several different problems, depending on the product. Oral diazepam (tablets such as Valium, and oral solution) is used for anxiety disorders, short-term relief of anxiety symptoms, and symptom relief in acute alcohol withdrawal. It is also used alongside other treatments for muscle spasm and, adjunctively, for convulsive disorders (seizure disorders).
Diazepam injection is used for the same anxiety and alcohol withdrawal uses. It is also used before endoscopic procedures, before surgery, and before cardioversion (when given intravenously). It helps with muscle spasm and tetanus, and with status epilepticus (a prolonged, dangerous seizure) as an add-on. Diazepam rectal gel and Valtoco nasal spray are used only for seizure clusters (acute repetitive seizures) in people with epilepsy ages 2 and older.

Diazepam Is Linked to Lower Levels of One Nutrient
Natural Medicines records Diazepam as a medicine that can lower these. This is not an interaction with Terminalia — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
