Levonorgestrel, Ethinyl Estradiol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Your birth control pill (Microgynon, Ovranette, and similar brands) is cleared from your body partly by a liver enzyme called CYP3A4. Lab and animal studies hint that Terminalia arjuna may slow that enzyme down a little. In theory, that could let the hormones in your pill linger a bit longer, which might nudge up estrogen-type side effects like nausea, breast tenderness, bloating, or headaches.

I want to be honest about how shaky this is. It comes only from test-tube and animal work (not from people taking both together), so we really don't know if it matters in real life. The good news is this direction doesn't point to your pill working less well. Still, before adding Terminalia, please check with your pharmacist or doctor, especially given your heart health.
