Miconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist: lab studies suggest Terminalia arjuna may block an enzyme called CYP3A4, which the body uses to break down many medicines. In theory, that could let those drugs build up a bit. Miconazole is technically on that list, but here's the reassuring part for most people.

Most miconazole products (like Monistat creams, suppositories, and antifungal powders) work right where you put them on the skin or in the vagina, with very little absorbed into your bloodstream. So a topical product is unlikely to interact in any meaningful way. Interestingly, miconazole itself is also a known CYP3A4 blocker, so any concern is more theoretical than proven. If you ever take an oral miconazole gel, it's worth a quick chat with your pharmacist.
