Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist for ivabradine (Corlanor): your body clears it using a liver enzyme called CYP3A4. Lab studies suggest Terminalia arjuna may slow that enzyme down. If it does, ivabradine could build up in your system and hit harder than intended.

For ivabradine, more drug usually means a slower heart rate than you want. That can show up as dizziness, tiredness, lightheadedness, or those temporary visual brightness flashes (phosphenes) the drug is known for. I'll be honest though, this is only theoretical and based on test-tube work, not real patients, so I wouldn't panic. Still, since heart-rate drugs leave little room for error, please check with your pharmacist or doctor before pairing them.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. About Ivabradine

What Is Ivabradine Used For?

Brand names: Procoralan, Coralan, Corlentor, Corlanor

Ivabradine (including the brand Corlanor) is used in adults with stable, symptomatic chronic heart failure with reduced pumping function to reduce the risk of being hospitalized for worsening heart failure. It is prescribed when the heart is beating too fast at rest and the patient is either already on the highest tolerated dose of a beta-blocker or cannot take one.

Corlanor is also approved to treat stable, symptomatic heart failure caused by dilated cardiomyopathy (an enlarged, weakened heart muscle) in children ages 6 months and older who have an elevated heart rate.
