Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version. Your liver uses an enzyme called CYP2C9 to break down ibuprofen and clear it out. Lab studies suggest that Terminalia arjuna may slow that enzyme down. If that happens in real life, ibuprofen could hang around longer and act a bit stronger than usual.
For ibuprofen, that mostly means a higher chance of its typical side effects, things like stomach upset, heartburn, an increased risk of stomach irritation or bleeding, and added strain on the kidneys or blood pressure. I want to be honest though: this is based only on test-tube research, not on people, so it's theoretical. Occasional ibuprofen is likely fine, but check with your pharmacist or doctor before pairing it with concentrated Terminalia supplements.

In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this pair. Ibuprofen, like other NSAIDs, already makes your platelets a little less sticky, which is part of why it can cause stomach upset, ulcers, or easier bruising and bleeding. Terminalia (especially the arjuna species) seems to do something similar in lab studies, gently slowing how platelets clump together to form clots.
Stack the two and you could, in theory, nudge up your bleeding risk a bit. The honest part: this concern comes from test-tube and animal research, not from people, so it's more of a caution than a proven problem. Still, if you notice unusual bruising, nosebleeds, or dark stools, or if you take other blood thinners, check in with your pharmacist or doctor before combining them.

Over-the-counter ibuprofen products (tablets, capsules and others) temporarily relieve minor aches and pains from headache, toothache, backache, muscular aches, the common cold, menstrual cramps and minor arthritis pain. They also temporarily reduce fever.
Prescription ibuprofen tablets are used for signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods). Caldolor (intravenous) is used in adults and children 3 months and older for mild to moderate pain, moderate to severe pain along with opioid medicines, and fever. Ibuprofen Lysine (intravenous) is used only to close a patent ductus arteriosus (PDA, a heart vessel that stays open) in certain premature infants.
