Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to break down diclofenac and clear it out. Lab studies suggest that Terminalia arjuna can slow this enzyme down. If that happens, diclofenac could hang around longer and build up a bit, which may nudge up its usual side effects, things like stomach upset, heartburn, a higher risk of stomach irritation or bleeding, fluid retention, or blood pressure changes.

I want to be honest about the strength of this, though. So far this is only seen in test-tube research, not in real people, so it's more of a theoretical concern than a proven problem. Still, since both can affect the heart and stomach, it's worth a quick chat with your pharmacist or doctor before combining them.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the gist: diclofenac is an NSAID, and like other pain relievers in that family, it can make your platelets (the cells that help blood clot) a little less sticky. That's part of why NSAIDs can sometimes cause stomach irritation and bleeding. Terminalia arjuna may do something similar, since lab studies suggest it can slow down platelets too. Pair the two and you could, in theory, nudge your bleeding risk a bit higher.

I want to be honest about the strength of this, though. The concern comes from test-tube research, not from people, so it's more theoretical than proven. Still, watch for things like easy bruising, nosebleeds, dark or bloody stools, or stomach pain. If you take diclofenac regularly, just check with your pharmacist or doctor before adding Terminalia.

Diclofenac sodium topical gel is used to treat actinic keratoses (AK) — rough, scaly skin patches that form after years of sun exposure. These patches are considered precancerous, meaning they can develop into skin cancer if left untreated.

This is a topical gel, meaning it is applied directly to the skin over the affected area rather than taken by mouth. It belongs to the NSAID (nonsteroidal anti-inflammatory drug) family.
