Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.

Here's the gist for your Foradil or Perforomist inhaler. Terminalia arjuna has been shown, but only in lab dish studies, to slow down a liver enzyme called CYP2D6. Since formoterol is partly cleared by that pathway, blocking it could in theory let a little more of the drug linger in your body. Formoterol is not a prodrug, so more drug would mean a slightly stronger effect.

In practice, that could show up as a faster or pounding heartbeat, jitteriness, shaky hands, or a headache. The honest truth is this is theoretical and based on test-tube work, not real patients, so it may not amount to much. Still, if you have a heart condition, please check with your pharmacist or doctor before combining them.

Here's the gist: lab studies suggest Terminalia arjuna can slow down a liver enzyme called CYP2C9. The idea is that if your body clears a drug more slowly, that drug can build up a bit and cause stronger effects. With Formoterol, that could theoretically mean more of the typical side effects this inhaler can cause, like a fast or pounding heartbeat, jitteriness, tremor (shaky hands), or a nervous feeling.

That said, this is a theoretical concern based only on test-tube research, not on real people, so I wouldn't lose sleep over it. Formoterol also isn't a prodrug, so it doesn't get flipped. Still, if you have a heart condition or notice extra palpitations or shakiness, check with your pharmacist or doctor before combining them.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution (nebulizer; sold as Perforomist and as generic formoterol fumarate) is used for long-term, twice-daily maintenance treatment of bronchoconstriction (tightening of the airways) in people with chronic obstructive pulmonary disease (COPD). This includes chronic bronchitis and emphysema.

It is not for sudden COPD flare-ups and not for asthma.
