Enasidenib falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: lab studies suggest Terminalia arjuna may slow down an enzyme called CYP2D6. Your leukemia medicine, enasidenib (Idhifa), is handled in part by that same enzyme. If Terminalia blocks it, enasidenib could build up a bit more than usual in your body, which might make side effects like nausea, low appetite, tiredness, or changes in your blood counts and bilirubin more likely.
I want to be honest about the strength of this, though. It comes only from test-tube research, not from people, so it's theoretical at this point. Since enasidenib is a serious cancer drug with careful monitoring, please don't add Terminalia without first checking with your oncologist or pharmacist.

Here's the basic idea with Terminalia (arjuna) and your Idhifa (enasidenib). Lab research suggests Terminalia may block a liver enzyme called CYP2C9. Enasidenib is partly handled by that same enzyme, and it is an active drug, not a prodrug. So if Terminalia slows that pathway, your body could clear enasidenib a bit more slowly, letting levels build up. In theory that might mean stronger effects and more side effects, such as nausea, diarrhea, tiredness, or rising bilirubin (yellowing of skin or eyes).
I want to be honest though: this is theoretical, based only on test-tube studies, not real patients. Still, because enasidenib is a serious leukemia medicine, please run any Terminalia supplement past your oncology team before starting it.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help clear enasidenib. Lab studies suggest Terminalia arjuna can slow that same enzyme down. In theory, that means enasidenib could build up a bit higher than expected, which might increase its effects and side effects, things like nausea, diarrhea, low appetite, or changes in liver and bilirubin labs. Enasidenib is an active drug, not a prodrug, so more drug hanging around generally means stronger effects, not weaker ones.
I want to be honest about the strength here: this is only test-tube evidence, so we don't truly know if it matters in real people. Since enasidenib is serious cancer therapy, please loop in your oncology team or pharmacist before adding Terminalia.

What Is Enasidenib Used For?

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
