Enasidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group St. John's Wort interacts with.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with pairing St. John's wort and your leukemia medicine, enasidenib (Idhifa). Your body partly clears enasidenib using a liver enzyme called CYP3A4. St. John's wort revs that enzyme up (it's an inducer), so it can push enasidenib out of your system faster and lower the levels in your blood.
For a cancer drug, that matters a lot. Lower levels could mean it doesn't fight your AML as well as it should, which is a real risk you don't want to take. This isn't about enasidenib being a prodrug; it's simply about losing the medicine's strength. Please don't start or stop anything on your own. Talk with your oncology team or pharmacist before using St. John's wort while on this treatment.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Preliminary clinical research in healthy males shows that taking St. John's wort for 14 days induces CYP2C19 and increases metabolism of mephenytoin (Mesantoin). In patients with wild-type 2C19 (2C19*1/*1) metabolism was almost 4-fold greater in subjects who received St. John's wort compared to placebo. In contrast, patients with 2C19*2/*2 and *2/*3 genotypes did not demonstrate a similar increase in metabolism. Theoretically, St. John's wort might increase metabolism of other CYP2C19 substrates.

Here's the concern with pairing St. John's wort and Idhifa (enasidenib). St. John's wort tends to speed up a liver enzyme called CYP2C19, and enasidenib is partly handled by that same enzyme. When the enzyme works faster, your body can clear the medicine more quickly, which may lower the amount in your blood. For a leukemia treatment, weaker levels could mean it doesn't fight the cancer as well as it should, and that's not a risk worth taking.
One thing to keep in mind: the evidence here comes from a small study using a different CYP2C19 drug, so the effect on enasidenib specifically is more theoretical than proven. Still, with a cancer medication, please don't start St. John's wort without first checking with your oncologist or pharmacist.

Here's the concern with pairing St. John's Wort and enasidenib (Idhifa). St. John's wort can speed up a liver enzyme called CYP1A2, which is one of the pathways your body uses to clear enasidenib. When that enzyme works faster, the drug can get cleared out more quickly, and its levels in your blood may drop.
Enasidenib is the active drug itself, not a prodrug, so lower levels likely mean a weaker anti-leukemia effect. With a cancer medicine, you really don't want to risk it working less well. This interaction is rated moderate and is considered possible rather than proven, but given what's at stake, please don't combine these without first checking with your oncologist or pharmacist.

There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

I'll be honest, the research here is mixed. St. John's wort affects CYP2C9 less strongly than some other enzymes, so the real-world impact isn't fully proven. Still, this is cancer treatment, so I wouldn't take chances. Please check with your oncologist or pharmacist before combining them.

Clinical research shows that taking St. John's wort 325 mg three times daily for 14 days along with bupropion, a CYP2B6 substrate, reduces the area under the concentration-time curve by approximately 14% and increases the clearance of bupropion by approximately 20%.

Here's the concern with pairing these two. St. John's wort tends to rev up certain liver enzymes, including one called CYP2B6, which is one of the pathways that helps process enasidenib (Idhifa). When that enzyme speeds up, your body may clear the drug a little faster, which could mean lower levels in your blood and possibly a weaker effect against your leukemia. With a cancer medicine, you really don't want anything quietly chipping away at how well it works.
Now, the direct evidence is for a different drug (bupropion), and the drop there was modest, around 14%, so this is more of a caution than a sure thing. Still, given the stakes with AML treatment, please don't add St. John's wort without first checking with your oncologist or pharmacist.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
