Why this applies to Enasidenib
Enasidenib falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Active Hexose Correlated Compound (ahcc) interacts with.
In vitro studies suggest that AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Idhifa (enasidenib) and AHCC. Your body partly uses a liver enzyme called CYP2D6 to handle enasidenib. In a test-tube study, AHCC seemed to speed that enzyme up. Notice I said test-tube. This has not been seen in real people, so it's a theoretical concern rather than something we know happens.
If it did occur, speeding up CYP2D6 could lower enasidenib levels, which might make this leukemia treatment work less well. That matters, because enasidenib is a serious cancer drug and you want full effect. The evidence here is weak, so don't panic. Just loop in your oncology team or pharmacist before adding AHCC so they can keep an eye on things.

It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
