Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.

Here's the basic idea with Terminalia (especially Terminalia arjuna) and your bosentan. Your body uses an enzyme called CYP2C9 to help clear bosentan out of your system. Lab studies suggest Terminalia may slow that enzyme down a bit. If it does, bosentan could clear more slowly and build up, which in theory means stronger effects and a higher chance of side effects like headache, flushing, swelling in your legs or ankles, low blood pressure, or changes in liver tests (something bosentan already requires monitoring for).

I want to be honest about the strength here: this is based only on test-tube research, not real people, so we don't truly know if it matters day to day. Still, since bosentan is a serious medication for your lungs, please loop in your pharmacist or doctor before adding Terminalia.

Your body relies on a liver enzyme called CYP3A4 to help clear bosentan (Tracleer). Lab studies suggest that Terminalia arjuna may slow this enzyme down. In theory, that could let bosentan build up a bit higher than usual, which might mean more of its typical side effects like headache, flushing, swelling in the legs or ankles, and feeling tired or lightheaded. Bosentan can also affect the liver, so anything that raises its levels is worth watching.

Keep in mind this concern comes only from test-tube research, not from studies in people, so we can't say how much it really matters in real life. Still, since bosentan is a serious medication for your lungs, please check with your pharmacist or doctor before adding Terminalia.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. Some small studies suggest possible benefits for heart and cholesterol health, but the evidence is limited and not strong enough to recommend it as a treatment. What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
