CNS DEPRESSANTS

Acetaminophen, Chlorzoxazone, Codeine falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combination product is worth a closer look. Two parts of it can calm the nervous system: the codeine (an opioid pain reliever) and the chlorzoxazone (a muscle relaxant). Kava also has calming, sedative effects of its own. Stack them together and those effects can add up, leaving you more drowsy, foggy, dizzy, or unsteady on your feet than you'd expect. In a stronger case, breathing and reflexes can slow down too.
There's another wrinkle here. Both the acetaminophen in this pill and kava have each been tied to liver strain, so using them together may add stress on the liver. Please don't stop or change anything on your own. Talk with your pharmacist or doctor before pairing kava with this medicine.

Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the main thing to know about pairing kava with this combination pill (which contains acetaminophen, chlorzoxazone, and codeine): the concern is your liver. Two of these ingredients already lean on the liver. Acetaminophen (Tylenol) is a well-known cause of liver injury when doses run high, and chlorzoxazone, a muscle relaxer, has also been tied to rare liver problems. Kava has its own track record of liver damage, mostly with heavy or long-term use.
Stacking them could, in theory, add to the strain on your liver. To be honest, this is based on scattered case reports rather than strong proof, so I don't want to alarm you. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please run this combo by your pharmacist or doctor first.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days inhibited the metabolism of CYP2E1 substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combo pill mixes acetaminophen (Tylenol), a muscle relaxer called chlorzoxazone, and codeine. The piece that matters here is chlorzoxazone, which your body clears using an enzyme known as CYP2E1. In a small human study, kava slowed that enzyme down. So if you take kava regularly, chlorzoxazone could build up a bit more than usual, which may leave you feeling extra drowsy, lightheaded, or sluggish.
There's another reason to be careful: both kava and acetaminophen can stress the liver, so using them together adds a little extra strain. The evidence here is real but limited, so I wouldn't panic. Please check with your pharmacist or doctor before pairing them, and don't change anything on your own.

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. Here's the honest scoop on this combo pill (acetaminophen, chlorzoxazone, and codeine) with kava. The worry on file is that kava might slow down a liver enzyme called CYP1A2, which could in theory let some medicines linger a bit longer. But the better-quality human research tells a calmer story: a 28-day study showed kava had no real effect on CYP1A2. So this particular interaction is rated minor and unlikely.
That said, I'd still raise a flag for a different reason. Both kava and the codeine in this product can make you drowsy, relaxed, and foggy, so combining them may add up to extra sleepiness or dizziness. Kava has also been tied to rare liver injury, and acetaminophen is hard on the liver too. Please check with your pharmacist or doctor before mixing these.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

This combination product contains codeine, which is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, the part that actually relieves pain. In test-tube studies, kava appeared to block CYP2D6, which in theory could mean less codeine gets activated and weaker pain relief. The good news: in actual human studies, kava did not meaningfully change how CYP2D6 handles its substrates, so this concern looks mostly theoretical.
What I'd watch more closely is overlap in calming effects. Both kava and codeine can make you drowsy, so together you may feel sleepier or foggier. Also, kava and the acetaminophen in this product can each stress the liver. Please check with your pharmacist or doctor before pairing them.
