Why this applies to Aspirin, Codeine Phosphate

Aspirin, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2D6.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the wrinkle with this combo: the codeine part is a prodrug. Your body has to convert codeine into morphine using an enzyme called CYP2D6 before you get much pain relief. Lab and animal studies hint that Fo-ti (especially the raw root) might slow that same enzyme down. If that happened in people, you could turn less codeine into morphine, meaning weaker pain relief rather than stronger.

I'll be honest though: this is theoretical and hasn't been seen in humans, so I wouldn't lose sleep over it. The aspirin half isn't really affected here. A bigger reason to check in with your pharmacist is that processed Fo-ti has been linked to liver injury, and aspirin has its own stomach and bleeding concerns. Run it by your pharmacist before combining them.

Aspirin, Codeine Phosphate also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Fo-ti has been linked to cases of acute liver failure which can decrease clotting factor production and increase the effects of anticoagulants. In one case, a patient who had been stable on warfarin presented with acute hepatitis and an INR elevated to 14.98. The patient had been taking fo-ti for 90 days prior to admission. Discontinuation of warfarin and fo-ti lead to a decrease in the INR and full recovery. Theoretically, concomitant use of fo-ti with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients. Until more is known, monitor patients taking fo-ti and drugs that affect bleeding.
Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), dipyridamole (Persantine), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the part that matters for your Aspirin with Codeine combo: the aspirin half already thins your blood a bit by making platelets less sticky. Fo-ti's concern isn't that it speeds up or slows down how you process this medicine. Instead, processed Fo-ti has been tied to rare cases of liver injury, and a damaged liver makes fewer of the proteins that help your blood clot. Stack that on top of aspirin, and the worry is a higher chance of bleeding problems, things like easy bruising, nosebleeds, bleeding gums, or dark, tarry stools.

I'll be honest, this is built on a few case reports (one was a warfarin patient), so the evidence is thin and theoretical for aspirin. Still, since the liver risk is real, please loop in your pharmacist or doctor before pairing them, and watch for any unusual bleeding.
