Why this applies to Fluticasone

Fluticasone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the gist with Fluticasone: your liver uses an enzyme called CYP3A4 to break it down. In lab dishes, Fo-ti has shown some ability to slow that enzyme. If that happened in your body, less fluticasone would get cleared, so more could build up and its effects could be stronger than expected.

What does that look like in real life? For the nasal spray (Flonase) or the inhaler (Flovent), the amounts absorbed are usually small, so this is rarely a big deal. But with higher steroid exposure over time you might see things like thrush in the mouth, easy bruising, or other steroid-type effects. Honestly, this is only theoretical and hasn't been seen in people, so I wouldn't lose sleep over it. Still, mention your Fo-ti use to your pharmacist or doctor just to be safe.
