Why this applies to Theophylline

Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Here's the thing about theophylline: your body relies on a liver enzyme called CYP1A2 to clear it out. Fo-ti may bump into that same enzyme, but the lab and animal studies are genuinely mixed. The raw plant and water-based extracts seemed to slow the enzyme (which could let theophylline build up), while an alcohol-based extract sped it up. None of this has actually been shown in people.

Why does this matter? Theophylline has a narrow safety window. If levels climb, you can get nausea, a racing heart, jitteriness, or even seizures. If they drop, your asthma or breathing could flare. Because this is still theoretical, don't change anything on your own. Just loop in your pharmacist or doctor before combining them.

What Is Theophylline Used For?

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
