Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

This combo pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and both already tend to make people sleepy. Kava brings its own calming, sedative effect to the table. Stack them together and those effects can add up, leaving you more drowsy, foggy, dizzy, or unsteady on your feet than you'd expect. That's a real concern with driving or with falls, especially in older folks.

Major medical groups specifically advise against mixing kava with sedating medicines, so this isn't just theory. There's also a separate worry: kava has been tied to rare liver injury, and you don't want to add liver stress on top of these meds. Please don't stop anything on your own. This combination product pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). Lab studies suggest kava can block an enzyme called CYP2C19, which is one of the pathways that helps clear amitriptyline from your body. In theory, less clearance means more amitriptyline hanging around, so you might notice stronger side effects like drowsiness, dry mouth, constipation, blurred vision, dizziness, or a fast heartbeat.

I want to be honest with you though: this is only seen in test-tube research and hasn't been shown in real people, so it's more of a caution than a proven problem. The bigger worry is that both kava and this medicine can calm the brain and stress the liver. Please check with your pharmacist or doctor before combining them, and don't change anything on your own.

In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Let me put your mind at ease a bit here. Triavil combines amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and the theory was that kava might slow down a liver enzyme called CYP1A2 that helps clear amitriptyline. If that happened, the drug could build up and you might feel more of its sedating side effects, like heavy drowsiness, dry mouth, dizziness, or constipation.

The good news is that the better-quality research shows kava does not really block this enzyme, so this interaction is rated minor and unlikely. The bigger thing I'd flag is that both kava and this medicine can be calming, so combining them may simply make you sleepier or groggier. Kava can also stress the liver. This combo pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and both lean on a liver enzyme called CYP2D6 to be cleared from your body. In a test tube, kava looked like it might block that enzyme, which in theory could let these medicines build up and bring on more drowsiness, dry mouth, constipation, blurry vision, or dizziness. The good news is that actual human studies did not show kava slowing down CYP2D6 in real life, so this is rated minor and unlikely.

My bigger concern is overlap, not metabolism. Kava is calming and these drugs are already sedating, so together they may make you extra sleepy. Kava can also rarely harm the liver. Please check with your pharmacist or doctor before pairing them.
