Why this applies to Ledipasvir, Sofosbuvir

Ledipasvir, Sofosbuvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist for Harvoni (ledipasvir and sofosbuvir). Both of these antivirals are carried around your body by a transporter called P-glycoprotein, or P-gp. In a test tube, kava's active compounds (kavalactones) seemed to slow that transporter down. If that happened in real life, more of your Harvoni could linger in your system, which in theory might nudge up side effects like tiredness, headache, or nausea.

The honest part: this is mostly lab-dish evidence, and a human study using another P-gp drug showed no real change. So the worry is more theoretical than proven. Still, kava itself can stress the liver, and you're treating a liver infection, so please check with your pharmacist or doctor before combining them. Don't change anything on your own.

What Is Ledipasvir and Sofosbuvir Used For?

Brand name: Harvoni

Ledipasvir and sofosbuvir treats chronic hepatitis C virus (HCV) infection in adults and pediatric patients 3 years of age and older. It is approved for HCV genotypes 1, 4, 5, and 6 in patients without cirrhosis or with compensated cirrhosis (mild-to-moderate liver scarring that the liver is still managing), and for genotype 1 with decompensated cirrhosis (advanced liver disease) when used together with ribavirin.

It is also approved for liver transplant recipients with HCV genotype 1 or 4 infection who do not have cirrhosis or have compensated cirrhosis, in combination with ribavirin. Both Harvoni and the generic ledipasvir and sofosbuvir tablets share these indications.
