In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with kava and clomipramine (Anafranil). Your body uses an enzyme called CYP2C19 to help break down clomipramine. In lab dish studies, kava appears to block that enzyme. If that same thing happened in people, clomipramine could clear more slowly and build up a bit, which might mean stronger side effects like drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat.
I want to be honest, though: this is only seen in test-tube research so far, not in actual patients, so it's more of a theoretical heads-up than a proven problem. There's also a separate concern, both kava and this medicine can stress the liver, so doubling up isn't ideal. Please loop in your pharmacist or doctor before combining them.

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the practical picture with kava and Clomipramine. Your body partly clears clomipramine using a liver enzyme called CYP1A2. The thought was that kava might slow that enzyme down, which could let clomipramine build up and bump up its usual side effects, things like drowsiness, dry mouth, constipation, blurry vision, or feeling lightheaded. But the better-quality human research found that kava did not meaningfully change CYP1A2 activity, so this interaction is considered minor and unlikely.
The bigger thing I'd flag isn't the enzyme. Both kava and clomipramine can be sedating, and kava has been tied to rare liver injury. So if you ever feel extra sleepy or notice yellowing skin or dark urine, loop in your pharmacist or doctor before changing anything.
